Coaching Agreement

Between Well with Nic (“Coach”) and Client (“Client”)

1. Purpose of Coaching

Coaching is a collaborative, client-centred process designed to facilitate and empower you to develop and achieve self-determined goals related to your health, wellbeing, and performance.

Coaching is distinct from therapy, counselling, or medical treatment. It does not diagnose, treat, or advise on clinical or psychological matters.

2. About Coaching

Coaching is a professional relationship, grounded in positive psychology, that supports and empowers you to stretch and grow beyond what you can do on your own, to become the healthiest, most effective version of yourself.
Through a structured inquiry, your coach helps you identify your vision, motivators, obstacles, and strategies to sustain long-term change.

3. The First Step – Your Intake Questionnaire

Before your first session, you will be asked to complete a brief intake questionnaire.

This helps define what is currently working well and what you would like to change moving forward. It forms the foundation for your initial goals and direction.

4. Coaching Philosophy and Scope

Coaching supports you in mobilising internal strengths, external resources, and self-management strategies to achieve sustainable behaviour change.

All coaching delivered under this agreement is conducted within the HCANZA Scope of Practice and consistent with any treatment plans prescribed by your Primary Care Physician or other Allied Health Professionals.

5. About Our Sessions

  • Sessions are typically either 30 minutes, or 45–60 minutes and can occur in person, online, or by phone by agreement.

  • Together, we’ll set realistic goals and maintain a steady course for success.

  • If you experience setbacks, they’ll be treated as part of the process.

  • Appointment times are by mutual agreement, and 24 hours’ notice is required for cancellations.

6. Coach Responsibilities

As your coach, I will:

  • Be punctual, responsive, and professional.

  • Maintain confidentiality and uphold HCANZA’s Code of Conduct.

  • Encourage realistic expectations and goals.

  • Provide direct and constructive feedback when helpful.

  • Support you in identifying creative solutions to overcome challenges.

  • Provide evidence-based information or resources when requested.

  • Ask permission before offering advice or direction.

  • Recognise when issues fall outside my professional scope and refer appropriately.

  • Ensure a clear understanding of the coaching process and responsibilities.

7. Client Responsibilities

As the client, you agree to:

  • Be punctual, responsive, and committed to your sessions.

  • Take responsibility for your progress and actions.

  • Be open and honest, sharing relevant information to support your goals.

  • Approach challenges with curiosity and self-awareness.

  • Be willing to try new approaches and practice self-compassion through setbacks.

  • Seek professional advice where required (e.g., medical, legal, or psychological).

  • Notify your coach promptly if a problem arises that could affect this agreement.

8. Terms of Service

  • Commencement Date: By mutual agreement. Coaching sessions are undertaken at your own discretion and responsibility.

  • Cancellations: 24 hours’ notice is required, or the session may be charged in full.

  • Invoices: Issued on 7-day payment terms.

  • Duration: Engagement may be ongoing or program-based as agreed.

9. Confidentiality and Privacy

All information shared is confidential and securely stored in compliance with privacy law.
Confidentiality may only be broken if required by law (e.g., risk of harm or court order).
You may authorise limited information sharing with other health professionals as needed.

10. Equality and Respect

Coaching is delivered in a manner that respects all clients regardless of age, gender, ethnicity, culture, religion, sexual orientation, or ability.

Both coach and client agree to maintain a relationship based on dignity, inclusion, and professional respect.

11. Termination of Coaching

Either party may terminate this agreement at any time. If coaching is no longer beneficial, the coach may refer you to another professional or service for continued support.

12. Acknowledgement

This agreement aligns with the HCANZA Scope of Practice (April 2023) and HCANZA Code of Conduct (April 2023).
